29 CFR 1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized: (a) At the project site on or about October 19, 2012, and at times prior thereto, the 30A, 600VAC, 250VDC Siemens Heavy Duty Safety Switch and 30A, 240VAC, Cutler-Hammer Safety Switch, was in the OFF position and locked. A tag was not attached in the area where such equipment can be energized.
