29 CFR 1926.602(c)(1)(vii): Unauthorized persons were allowed to ride on a powered industrial truck: (a) On October 19, 2012, at a roadway construction project located along Main St in Grain Valley, MO., an employee was exposed to life threatening struck-by /crushing injuries, in that he was observed riding on the access steps of a Caterpillar 928-G Wheel Loader as it traveled approx. one half mile down a business street. Abatement certification for this violation is not required.
