29 CFR 1926.453(b)(2)(iv): Employee working in an aerial lift were not standing firmly on the floor of the basket:  The employer is failing to protect employees from fall hazards. This was most recently documented on December 2, 2016 at Tudor Road between NW Ward Road and NE Douglas Street in Lees Summit, Missouri 64086. An Employee was working in an aerial lift approximately 35 feet off of the ground when the employee exited the aerial lift onto a platform without the use of any fall protection. . The employer failed to provide training for the employee on maintaining two feet on the floor of the basket at all times.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  The employer is failing to protect employees from fall hazards. 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  The employer is failing to protect employees from hazards on the jobsite by not providing proper training. This was most recently documented on December 2, 2016 at Tudor Road between NW Ward Road and NE Douglas Street in Lees Summit, Missouri 64086. The employees were not properly trained by a qualified person to work from an aerial lift.
