29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)        a) North side - behind the patio - 2nd level - an employee was exposed to a fall of 13 feet while taking measurements and cutting lath. The scaffold was not fully planked, in that, it was 40 inches wide with one 10 inch wood plank and one 19 inch metal plank.     b) West side - 2nd level -  an employee was exposed to a fall of 13 feet 3 inches while using the scaffold to determine if the stucco was dry. The scaffold was not fully planked, in that, it was 40 inches wide with three 10 inch wood planks.     c) West side - 3nd level -  an employee was exposed to a fall of 19 feet 7 inches while using the scaffold to determine if the stucco was dry. The scaffold was not fully planked, in that, it was 40 inches wide with one 10 inch wood plank and one 19 inch metal plank.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.        a) North side - behind the patio - 2nd level - an employee was exposed to a fall of 13 feet while using the end frames to access and egress the scaffold.      b) West side - 2nd level to the 3rd level -  an employee was exposed to a fall of approximately 6 feet while using the end frames to access and egress the scaffold.

29 CFR 1926.451(g)(1):   Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:      a) On or about October 19, 2012, an employee working from the 2nd level of a tubular scaffold where no guardrail system was being used exposing him to a fall of 13 feet hazard while taking measurements and cutting lath.      b) On or about October 19, 2012, an employee was exposed to a fall of 13 feet 3 inches while using the scaffold to determine if the stucco was dry. There was no guardrail system on the north end of the scaffold.     c) On or about October 19, 2012, an employee was exposed to a fall of 19 feet 7 inches while using the scaffold to determine if the stucco was dry. There was no guardrail system on the north end of the scaffold.
