29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a)  Hillside Builders, LLC on or about October 22, 2012, at the site address of Sub Lot 120 Long Cove, Avon Lake, OH: The employer did not ensure employees were adequately protected from falls while performing framing activities atop the second story exterior top plate of the residential structure.  During this work, employees were exposed to falls of approximately 18 ft. to the ground below.    b)  Hillside Builders, LLC on or about October 22, 2012, at the site address of Sub Lot 120 Long Cove, Avon Lake, OH: The employer did not ensure an employee was adequately protected from falls while performing sheeting activities atop the second story gable on the west side of the residential structure.  During this work, the employee was exposed to falls of approximately 18 ft. to the ground below.    Hillside Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 282324, citation number 1, item number 2 and was affirmed as a final order on May 17, 2012, with respect to a workplace located at Sub Lot 91 Westport Pl. Avon Lake, OH.
