29 CFR 1926.404(a)(2):     A grounded conductor was attached to a terminal or lead so as to reverse designated polarity:    a) On or about 10/16/12, at the Moog Inc. Corporate Headquarters Construction Site, Elma, NY: Employees of one contractor used a DeWalt Compound Mitre Saw to cut metal parts. Employees of another contractor used flourescent light fixtures to provide lighting in an elevator shaft and to power other equipment. The Mitre Saw and flourescent light fixtures were connected to 120 volt-ac power via a series of extension cords, the first of which was plugged into a  receptacle in a 4-receptacle outlet box mounted on a wooden 2x4 in the lower level main office area inside the building. The first cord in the series was a short black GFCI 3-Way. All 3 receptacles on this cord had "reversed polarity". This defective cord was marked with a magic marker as "Jacks", and belonged to K.T. Construction Services, Inc.    NO ABATEMENT CERTIFICATION REQUIRED
