29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) On or about 10/16/12, at the Moog Inc. Corporate Headquarters Construction Site, Elma, NY: Employee used a DeWalt Compound Mitre Saw to cut metal parts. The Mitre Saw was connected to 120 volt-ac power via a series of two extension cords, the first of which was plugged into a receptacle in a 4-receptacle outlet box mounted on a wooden 2x4 in the lower level main office area inside thebuilding. The first cord in the series was a short black GFCI 3-Way which belonged to another contractor. All 3 receptacles on this cord had "reversed polarity". NO ABATEMENT CERTIFICATION REQUIRED

Corporate Headquarters Construction Site, Elma, NY: Employee used a DeWalt Compound Mitre Saw to cut metal parts. The Mitre Saw was connected to 120 volt-ac power via a series of two extension cords, the first of which was plugged into a receptacle in a 4-receptacle outlet box mounted on a wooden 2x4 in the lower level main office area inside thebuilding. The first cord in the series was a short black GFCI 3-Way which belonged to another contractor. All 3 receptacles on this cord were "ungrounded" because the grounding prong was missing on the plug of this cord. NO ABATEMENT CERTIFICATION REQUIRED
