29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Employee(s) operating the 50 ton HDC hydraulic press brake in the Fabrication Department were exposed to physical injuries due to no guard being provided for the metal bending area of the press brake. (b) Employee(s) operating the 135 ton Cincinnati hydraulic press brake in the Fabrication Department were exposed to physical injuries due to no guard being provided for the metal bending area of the press brake. (c) Employee(s) operating the 350 ton Cincinnati hydraulic press brake in the Fabrication Department were exposed to physical injuries due to no guard being provided for the metal bending area of the press brake. (d) Employee(s) operating the 90 ton Cincinnati hydraulic press brake in the Fabrication Department were exposed to physical injuries due to no guard being provided for the metal bending area of the press brake. (e) Employee(s) operating the HTC hydraulic press brake in the Fabrication Department were exposed to physical injuries due to no guard being provided for the metal bending area of the press brake. (f) Employee(s) operating the Willis RD-1700H radial drill between 663 - 796 rpms in the Fabrication Department were exposed to physical injuries due to the current guard not enclosing the rotating chuck and point of operation. (g) Employee(s) operating the Vanguard radial drill at 250 rpms in the Fabrication Department were exposed to physical injuries due to the current guard not enclosing the rotating chuck and the point of operation. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. Employee(s) operating the Iron worker 50 ton punch press in the Fabrication Department were exposed to physical injuries due to no guarding being provided to prevent the operator's hand from accessing the point of operation from the left / right sides or back side of the punch press.
