(a) Roof located at 753 Park Manor Drive, Smyrna, GA 30080: Employee was exposed to eye injuries while operating a Hitachi (Model NV45AB2) Coil Roofing Nail Gun installing ridge vent and roofing caps without safety glasses or other means of eye protection.

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:    (a) Inside pick-up truck located on the garage side of residential home at 753 Park Manor Drive, Smyrna, GA 30080: Employees were exposed to amputation hazards while operating a RIGID (Model GP90135) Wheelbarrow Air Compressor without the belt-guard in place. Employees were exposed to the moving parts as they worked in close proximity to the unguard equipment.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Roof located at 753 Park Manor Drive, Smyrna, GA 30080: Employee was exposed to fall hazards of approximately 20'7" on a 8/12 pitched roof while installing Timberline 30yr. Shingles while not attached to the fall protection system provided.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    (a) Garage side of the residential home located at 753 Park Manor Drive, Smyrna, GA 30080: Employees were exposed to falls of approximately 20'7" while using a material load ladder (TranzSporter Model LH400) with cable running through the middle and first 3' of the ladder obstructed by a loading platform as a means of access to the roof. The manufacturer of the ladder states that it should not be used as a personal ladder.
