Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) LCM Associates / 912 West 12th Street, Erie, PA: On or about October 22, 2012 personnel were conducting roofing operations for Umbrella Roofing Systems, Inc. on a flat roof approximately 15 feet from ground level. Personnel were not protected from falling by any fall protection systems.

The ladder extended 1 foot above the landing zone.
