29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) Job site, 4680 Old William Penn Highway, Monroeville, PA: On or about June 5, 2013, during roofing activities at a front corner area of the building, the employer did not ensure that safety monitor systems were properly used in that employees, working outside the warning flag areas, were performing roofing work activities while also acting as the safety monitor, and were exposed to fall hazards of up to approximately 30 feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Job site, 4680 Old William Penn Highway, Monroeville, PA: On or about June 5, 2013, during roofing activities, the employer did not ensure that fall protection systems/devices were used in that an employee worked outside the warning flag line to direct the crane loading materials on the roof at the rear of the building and had no safety monitor, harness, guardrails, safety nets or other fall protection in use where exposed to an approximate 30 foot fall hazard. L C M Associates Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 703199, citation number 1, item number 1, and was affirmed as a final order on February 26, 2013, with respect to a workplace located at 912 West 12th Street, Erie, PA.
