Employees were exposed to a fall distance of over 9 feet. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 17 feet above the ground. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. Ornat Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA violation number 314604554, citation 1, item 2, issued on July 8, 2010 and became final order on or about July 21, 2010 with respect to a workplace located at 7952 N. Octavia, Niles, IL 60714, and OSHA violation number 110553, citation 2, item 1, issued on December 22, 2011 and became final order on or about January 18, 2012 with respect to a workplace located at 1815 S. Fernandez, Arlington Heights, IL 60005.
