29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) In the maintenance shop, employees were exposed to the rotating chucks on the drill press JET 420; Mfg No 39754 which was not guarded and could operate up to 2400 rpm.

29 CFR 1910.265(c)(18)(i): The operation of conveyors was not be in accordance with American National Standard B20.1-1957 which states that pulley(s), sprocket(s), sheave(s), drum(s) and block(s) of conveyor(s) be arranged to prevent the possibility of injury due to hands or parts of clothing being caught between the belt(s) and pulley(s) or chain(s) and sprocket(s) or cable(s) and sheave(s), drum(s) or block(s):    (a) In the Stave Mill, at the top of the stairs used to access to the platform near the resaws, the belt and pulley on the conveyor belt to the resaw was not guarded to prevent the possible injury to employees hands or from clothing being caught between the belt and pulley.     (b) In the Stave Mill at the conveyor from the quarter saw to the resaw feed, the chain and sprocket on the conveyor was not guarded where the employee stood.  The unguarded chain and sprocket created a caught between hazard to employee hands and/or clothing.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent and continuous:    (a)  In the Stave Mill, near the resaws, two Stanley/Lasko fans were supplied power using at least two prong household extension cords and adapters.  The adapters bypassed the grounding of the fans creating an electrical shock hazard to employees.
