OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when they were exposed to struck-by and caught-in-between hazards:      On or about 10/10/12, at the above addressed jobsite, employees were exposed to the risk of being struck by collapsing pre-cast structural members due to not ensuring that column B-3 was properly grouted.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow and implement the Coreslab Erection Drawing E0.0  General Notes, Section 3.2 Welding, which requires welding of connections shall follow closely behind the erection of units.  Also, ensure Section 3.4 Grout and Solar Erectors erection procedure in the column section, item #4 are followed.         NOTE: ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED FOR THIS ITEM.
