29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about October 18, 2012, located in the pour mold department the employee engaged in the pouring of hot metal into floor mold(s) was not fully protected from burns with the personal protective equipment provided. The spats worn to protect the employees lower legs extremities did not completely cover and protect the back of the legs of the employee in that when worn they left a one inch to an inch and half gap open on the backside of the spats. The employee was exposed to burns in the event the previous mold poured had a unexpected leak or blew out while the employee had their back turned to that mold while pouring next mold in series.
