29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    Garrison Safety:  On or about 23 October 2012 and times prior thereto, the Safety office did not implement and provide a Energy Control/Isolation program to personnel working on the base who were performing electrical work and other maintenance activities.  This condition exposed employees to electrical shock, burns and caught in hazards.

Garrison Safety:  On or about 23 October 2012 and times prior thereto, the Safety office did not implement and provide training on the Lock Out-Tag Out program to personnel.  This condition exposed employees to electrical shock, burns and caught in hazards.

29 CFR 1960.7(c)(3):     Safety and health sampling, testing, and diagnostic and analytical tools and equipment, including laboratory analyses were not provided.    Safety Office:  On or about 23 October 2012 and times prior thereto, Safety personnel were not provided an electrical tester to analyze electrical circuits.  This exposed employees to electrical shock and burn hazards.
