29 CFR 1910.23(e)(3)(iv):     The anchoring of posts and framing of members for railings of all types shall be of such construction that the completed structure shall be capable of withstanding a load of at least 200 pounds applied in any direction at any point on the top rail.      DPW, Electrical/Plumbing Section:  On or about 23 October 2012 and times prior thereto, the mezzanine railing system was inadequate to restrict 200 lbs. of weight against it.  Mezzanine was 114 inches from the shop floor.  This condition exposed employees to fall hazards.

29 CFR 1910.213(g)(2):     Swing cutoff saw(s) were not provided with an effective device to return the saw automatically to the back of the table when released at any point of its travel.    DPW, Carpenter Shop,:  On or about 23 October 2012 and times prior thereto, the 120 volt, 860-M Panel Saw (FG0000031) would not retract to starting position when released.  This exposed employees to caught in hazard.

29 CFR 1910.215(a)(4):     Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.    DPW, Electrical/Plumbing shop:  On or about 23 October 2012 and times prior thereto, The work rest on the Delta  Bench Grinder, Serial Number 000134, measured 3/8 inches from the abrasive wheel.  This condition exposed employees to struck by hazards.

DPW, Electrical/Plumbing shop:  On or about 23 October 2012 and times prior thereto, The tongue guard on the Delta  Bench Grinder, Serial Number 000134, did not have a tongue guard installed.  This condition exposed employees to struck by hazards.    The U.S. Army, Fort Richardson Garrison was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307506915, Citation 02, Item Number 05, issued 09 December 2010, with respect to a workplace located at Fort Richardson, Alaska.

DPW, Carpenter Shop:  On or about 23 October 2012 and times prior thereto. a 120 volt, 2-plug junction box was wired to an extension cord.  This exposed employees to a 120 volt electrical shock hazard.     The U.S. Army, Fort Richardson Garrison was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307502443, Citation 02, Item Number 01, issued 17 January 2008, with respect to a workplace located at Fort Richardson, Alaska.
