29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    On or about 29 October 2012, and at times prior thereto, two workers were performing plaster work on a commercial building under construction while on a two-tier tubular-welded frame scaffold that was not fully planked, exposing the workers to a fall hazard of approximately 7 feet, 2 inches for one, and 13 feet, 9 inches for the other.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure scaffold platforms are fully planked from the front uprights to the back uprights where work is being performed.

29 CFR 1926.451(c)(3):     Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:    On or about 29 October 2012, and at times prior thereto, a worker was performing plaster work on a commercial building under construction while on a two-tier tubular-welded frame scaffold that was not braced at the level and location of the work, exposing the worker to a fall hazard of approximately 7 feet and 2 inches to the ground below.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure scaffold sections are provided with cross-bracing to ensure the stability of the system.

29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    On or about 29 October 2012, and at times prior thereto, a worker was performing plaster work on a commercial building under construction while on a two-tier tubular-welded frame scaffold that did not have fall protection at the end of the second tier level, exposing the worker to a fall hazard of approximately 13 feet and 9 inches to the ground below.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure fall protection, such as a gate or guardrails, are utilized in all areas where workers may be located.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about 29 October 2012, and at times prior thereto, two workers were performing plaster work on a commercial building under construction while on a two-tier tubular-welded frame scaffold that did not have a ladder for access, exposing the workers to a fall hazard of approximately 7 feet, 2 inches for one, and 13 feet, 9 inches for the other.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure a scaffold ladder is utilized for accessing the scaffold system.
