29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly: DOL Shipping and Receiving Area: On or about 24 October 2012, and times prio thereto, two fire extinguishers in the receiving area of the warehouse had not been inspected since July 2012; exposing employees to fire and smoke inhalation hazards. The U.S. Army, Fort Greely Garrison was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard whcih was contained in OSHA inspeciton # 307506865, Citation 1 , Item 4(a), issued 9 December 2010 with respect to a workplace located at Fort Greely, Alaska.

29 CFR 1910.134(c)(2)(i): The employer did not evaluate whether voluntary respirator use created a hazard and train employees on the voluntary use of respirators contained in Appendix "D" of this part. DOL Shiiping and Receiving: On or about 24 October 2012, and times prior thereto, employees were wearing Gerson # 1710 disposable respirators without having an evaluation performed to determine if respirators were not creating a hazard. In addition, employees were not trained on Appendix "D" of the standard; exposing employees to pulmonary hazards.
