29 CFR 1910.132(a):     Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.    (a)  POL Distribution Facility:  On or about 24 October 2012 and times prior thereto, Employees were standing on top of a refueling a truck without fall protection.  Working surface on top of truck was 102 inches above the ground.  This condition exposed employees to fall hazards.    (b)  POL Distribution Facility:  On or about 24 October 2012 and times prior thereto, Employees performing fueling operations were not provided summer weight flash retardant clothing for performing fueling/refueling operations.  Fuel being dispersed is gasoline, diesel and aviation fuel.  This condition exposed employees to burn hazards.

POL Distribution Facility:  On or about 24 October 2012 and times prior thereto, an Amerex portable fire extinguisher located in the pumping island was not inspected monthly.  This condition exposed employees to smoke inhalation and fire hazards.       The U.S. Army, Fort Greely Garrison was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307506865, Citation 01, Item Number 04, issued 09 December 2010, with respect to a workplace located at Fort Greely, Alaska.
