29 CFR 1910.37(a)(4): Safeguards designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting) must be in proper working order at all times. DOL Maintenance Facility: On or about 24 October 2012, and times prior thereto, the emergency lighting for exiting the facility was not operative in the following areas; exposing employees to fire and or smoke inhalation hazards in the event of an emergency evacuation. a) EL 6 in the machine shop. b) EL 2 and EL 3 in the main work bay area.

29 CFR 1910.145(f)(3): Tags shall be used as a means to prevent accidental injury or illness to employees who are exposed to hazardous or potentially hazardous conditions, equipment or operations which are out of the ordinary, unexpected or not readily apparent. Tags shall be used until such time as the identified hazard is eliminated or the hazardous operation is completed. Tags need not be used where signs, guarding or other positive means of protection are being used. DOL Maintenance Facility: On or about October 24 2012, and times prior thereto, a Cyclone DL 1500 blast cabinet was not tagged out of service; the gloves on the blast cabinet were ripped and not serviceable; exposing employees to silica hazards in the event it was used in that condition.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. DOL Maintenance Facility: On or about 24 October 2012, and times prior thereto, the emergency eyewash for the shop was not maintained in that the fluids had not been changed out for over 1 year; exposing employees to eye injury hazards.
