29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through. Warehouse Area: On or about 23 October and times prior thereto, the drainage hole by the ice making machine was 3 inches wide with no grating installed. This condition exposed employees to a tripping hazard.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel. (a) Rear Warehouse area: On or about 23 October 2012 and time prior thereto, the Facility Main breaker Control Panel MDP-1 was blocked with pallets of products impeding access to the circuit breaker panel area. This exposed employees to an electrical shock hazard. (b) Forward Warehouse area: On or about 23 October 2012 and times prior thereto, the Heater fans disconnect panel was blocked by pallets of store products. This exposed employees to an electrical shock hazard. The Defense Commissary was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307505396, Citation 02, Item Number 01, issued 21 January 2010, in respect to a workplace located at Elmendorf AFB, Alaska.

Warehouse Area: On or about 23 October 2012 and times prior thereto, (a) 120 volt, electrical panel DD had an open blank on the circuit breaker panel. This condition exposed employees to electrical shock and burn hazards. (b) 240 volt, electrical panel MDP-1 had an open area on the circuit breaker panel. This condition exposed employees to electrical shock and burn hazards. The Defense Commissary was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307505396, Citation 02, Item Number 02, issued 21 January 2010, in respect to a workplace located at Elmendorf AFB, Alaska.
