29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    Warehouse Area:  On or about 23 October and times prior thereto, the drainage hole by the ice making machine was 3 inches wide with no grating installed.  This condition exposed employees to a tripping hazard.

29 CFR 1910.37(a)(3):       Exit route(s) were not kept free and unobstructed:      Commissary Warehouse area:  On or about 23 October 2012 and times prior thereto, the exit route in backside of the freezer area was obstructed for employees to exit during an emergency.  This exposed employees to delays in exiting the building resulting in smoke inhalation and/or fire hazards.

29 CFR 1910.37(a)(4):     Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times    Commissary Administrative Area, 2nd floor:  On or about 23 October 2012 and times prior thereto, the emergency lighting in the administrative area of the second floor was inoperative.  This condition exposed employees to delays in exiting the building resulting in smoke inhalation and/or fire hazards.

29 CFR 1910.305(j)(2)(vii):     Receptacles installed in wet locations where the product intended to be plugged into it would be attended while in use (for example, portable tools) did not have an enclosure that was weatherproof when the attachment plug cap was removed.    Meat processing Room:  On or about 23 October 2012 and times prior thereto, an electrical outlet was missing the waterproof cover.  This condition exposed employees to an electrical shock hazard.

29 CFR 1910.304(f)(1)(iv):     Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.    (a)  Rear Warehouse area:  On or about 23 October 2012 and time prior thereto, the Facility Main breaker Control Panel MDP-1 was blocked with pallets of products impeding access to the circuit breaker panel area.  This exposed employees to an electrical shock hazard.           (b)  Forward Warehouse area:  On or about 23 October 2012 and times prior thereto, the Heater fans disconnect panel was blocked by pallets of store products.  This exposed employees to an electrical shock hazard.           The Defense Commissary was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307505396, Citation 02, Item Number 01, issued 21 January 2010, in respect to a workplace located at Elmendorf AFB, Alaska.

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed.    Warehouse Area:  On or about 23 October 2012 and times prior thereto,     (a) 120 volt, electrical panel DD had an open blank on the circuit breaker panel.  This condition exposed employees to electrical shock and burn hazards.      (b) 240 volt, electrical panel MDP-1 had an open area on the circuit breaker panel.  This condition exposed employees to electrical shock and burn hazards.      The Defense Commissary was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307505396, Citation 02, Item Number 02, issued 21 January 2010, in respect to a workplace located at Elmendorf AFB, Alaska.
