29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    Community Activity Center, Mechanical Room:  On or about 25 October 2012 and times prior thereto, two drain holes (measuring 11x11 inches) were partially covered with grating.  Opening measured 4 1/2 X10 1/2 inches.  This exposed employees to a tripping hazard.

29 CFR 1910.23(c)(1):     Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings on all open sides.    Community Activity Center, Bowling Lane Maintenance Area:  On or about 25 October 2012 and times prior thereto, there was no gate attached to the top of the twelve foot landing.  This condition exposed employees to a falling hazard.

29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    Community Activity Center, Bowling lane Mechanical Area:  On or about 25 October 2012 and times prior thereto, employees maintaining the Brunswick bowling equipment did not have a energy control lock out system in place.  This condition exposed employees to caught in, struck by hazards.

29 CFR 1910.176(a):     Aisles and passageways were not kept clear and in good repair, with no obstructions across or in aisles that would create a hazard.    Community Activity Center, Bowling Lane Work Area:  On or about 25 October 2012 and times prior thereto, the aisle in the maintenance area was blocked with a 14 foot step ladder.  This condition exposed employees to delays in existing the building causing smoke inhalation and fire hazards.
