29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a) Employees were exposed to the hazard of falling approximately 17 feet while working from scaffolding without the benefit of fall protection. Renaissance Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i) which was contained in OSHA inspection number 312110414, citation 1, item 6a, and was affirmed as a final order on July 17, 2008, with respect to a worksite located at 175 Sheridan Road, Winnetka, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501: a) Employees were exposed to the hazard of falling while working from a roof having a pitch of 12/12 and a ground to eave height of approximately 19 feet. Renaissance Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 392162, citation 1, item 1, and was affirmed as a final order on or about May 22, 2012, with respect to a worksite located at 846 Jackson, River Forest, Illinois. Renaissance Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 313112005, citation 1, item 3, and was affirmed as a final order on July 28, 2009, with respect to a worksite located at 3438 Pestalozzi Street, Saint Louis, Missouri. Renaissance Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 312110414, citation 1, item 6b, and was affirmed as a final order on about July 17, 2008, with respect to a worksite located at 175 Sheridan Road, Winnetka, Illinois.
