At the site located at 9555 Baythorne, Houston, TX; The employer failed to maintain OSHA 300 Logs. An employee sustained an injury on 9/15/2012 when he fell off a scoffold and an ambulance responded to the scene to provide medical attention, this injury should have been recorded on OSHA's 300 logs or equivalent forms.
