Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to an amputation hazard:  Inside the shop, the following press brakes were not equipped with protected foot controls to prevent actuation from falling or moving objects, or from unintended actuation:  a. Chicago Dreis & Krump 90-Ton press brake (no SN found)  b. Chicago Dreis & Krump 160-Ton press brake (SN #L17705)  c. Chicago Dries & Krump #4 press brake (Punch) (60-Ton, SN #1033A)    One method of abatement to correct this apparent hazard is to provide a protective cover over the foot pedal to prevent unattended machine actuation, as required by ANSI B11.3-2002, lparagraph 6.10.4.    DISCLAIMER:    a. The employer is not limited to the abatement method(s) suggested by OSHA.  b. The employer is responsible for selecting and implementing an effective abatement method.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to an amputation hazard:  Inside the shop, approximately thirty dies were not provided with the necessary information, including but not limited to:  a. rated press brake capacity requirements for the die;  b. weight of the upper die and other ram attachemnts required for the job set-up, when needed for setting counterbalance air pressure;  c. total die weight;  d.die space    One method of abatement to correct this apparent hazard is to provide the information required fot the die setters to  properly perform their tasks to be stamped on the face of the die or maintained in a file that is readily available to die setters, as required by ANSI B11.3-2002, paragraph 9.1.3.1. 29 CFR 1910.23(c)(1)  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  In the shop, for employees who climbed a ladder and retrieved files/boxes from the overhead storage on the shop office roof, a standard railing or equivalent was not present.  The fall distance was approximately 12 feet.
