29 CFR 1910.22(a)(2):     The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.    Instance a:     In the A house to C house crossover, there was a sheet of steel plating placed on the floor over an area where a conveyor was removed and there was a 2 3/4 gap beween the floor and the top of the sheet of metal thereby exposing employees to a trip/fall hazard.    Instance b:      In the center areas between the grain houses next to the dust systems, the doorway had unstable blocks stacked on the concrete to aid employees walking through water thereby exposing employees to a a trip/fall hazard.

29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    a.     In the A House Head pulley area on Bin 31, the cover was such that when stepping on the cover the weight caused the cover to go all the way through since there was no grate thereby exposing employees to a trip/fall hazard.

29 CFR 1910.23(e)(7)(iii):     The construction of floor opening covers may be of any material that meets the strength requirements. Covers projecting not more than 1 inch above the floor level may be used providing all edges are chamfered to an angle with the horizontal of not over 30 degrees. All hinges, handles, bolts, or other parts shall set flush with the floor or cover surface.    a.     In C House gallery area where employees off load the product from trucks to conveyors, the aiseway along side the conveyors hade covers to the bins which extended above the floor 1 1/4 inches to 1 1/2 inches thereby exposing employees to a trip/fall hazard.

29 CFR 1910.24(f):     "Stair treads." All treads shall be reasonably slip-resistant and the nosings shall be of nonslip finish. Welded bar grating treads without nosings are acceptable providing the leading edge can be readily identified by personnel descending the stairway and provided the tread is serrated or is of definite nonslip design. Rise height and tread width shall be uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs.    a.     On the northside of the B House, the stairs were bent and defromed and had physical damage thereby exposing employees to a trip/fall hazard.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a.     In the D house, the guard to the tripper wheel was not in place where the conveyor rotated thereby exposing employees to a caught-in hazard.    b.     In the C House on the south side of the building, there was a 1 1/2 inch opening in the side of the fan thereby exposing employees to a conntact hazard.    c.      In the C House, there was a 9 1/2 inch by a 9 inch opening in the guard to the head pulley and conveyor thereby exposing employees to a caught-in hazard.

Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.    a.     In the A House, the metal guarding around the head pulley was pulled away and not attached thereby exposing employees to a caught-in hazazrd.    b.      In the D House, beside the North Head Pulley, the shaft guard was not secured from displacement over the shaft to the conveyor moter thereby exposing employees to a caught-in hazard.

29 CFR 1910.219(c)(2)(i):     All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    a.     In the A house head pulley, the fill belt had an opening to the shaft which measured 12 1/2 inches by 6 inches thereby exposing employees to a rotating shaft and a caught-in hazard.    b.      In the C house, the dust fan located outside the door of the house had an opening to the shaft of 1 1/2 inches thereby exposing employees to a caught-in hazard.    c.       In the basement between Pit 2 and 3, the shaft was not properly guarded in that there was 1 1/2 inches between the casing and the shaft and six inches to the shaft thereby exposing employees to a caught-in hazard.

Each disconnecting means required by this subpart for motors and appliances shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.    a.     On Level 5, B house stripper, the disconnect to the tripper moter was not marked with the equipment it operated and the purpose was not clear thereby exposing employees to a contact hazard.
