29 CFR 1910.136(a):  The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:    a) Machine and Fabrication Shops: Employees operating overhead hoists were exposed to struck-by hazards because there was no requirement to wear protective footwear when handling steel plates.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Machine and Fabrication shop: Employees were exposed to mechanical energy, pneumatic energy, and electrical hazards while performing maintenance and servicing on equipment such as, but not limited to: Computer Numerical Controlled (CNC) vertical lathes, cranes and compressors, because the employer had not developed an energy control program.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, and rotating parts:    a) Machine Shop: Employees were exposed to caught-in and struck by hazards while operating a bed type milling machine. The machine was not guarded to protect employees from the rotating parts or flying objects.      b) Fabrication Shop: The employer exposed an employee operating a hydraulic press brake to a caught-in hazard. The press  was not guarded at the point of operation, and no safe work procedures were in place.
