29 CFR 1910.147(c)(1):  Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.      a.  On or about 11/8/12, at the CNC mill department, Vero Machine Industries Inc. The employer had not established an energy control program for protecting employees from caught by and electrical hazards while cleaning up and maintaining milling machines.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    At Vero Machine Industries Inc.       a. At the deburring department, an employee was exposed to a caught by hazard while working with a drill press (i.e. Grizzly Industrial, Model G7944) that lacked a guard on its point of operation, on or about 10/31/12.      b. At the manual machinery department, an employee was exposed to a caught by hazard while working with a lathe (i.e. Meuser & Co MOL-36965) that lacked a guard on its point of operation, on or about 10/31/12.      c. At the manual machinery department, an employee was exposed to a caught by hazard while working with a lathe (i.e. Hardinge Super-Precision) that lacked a guard on its point of operation, on or about 10/31/12.     d. At the manual machinery department, an employee was exposed to a caught by hazard while working with a belt sander that lacked a guard at pinch point areas, on or about 11/08/12.
