29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a.  On or about April 24, 2013, at the above addressed jobsite. An employee was exposed to an electrical hazard while operating circuit breakers inside a panelboard (QD Load Center Cat No. QOC442L) with an unused opening not covered.   b.  On or about April 24, 2013, at the above addressed jobsite. An employee was exposed to an electrical hazard while operating circuit breakers inside a panelboard (Siemens Load Center G244ML3200CU) with an unused opening not covered.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a.  On or about April 24, 2013, at the above addressed jobsite. The employer had not established an energy control program for protecting employees from caught by and electrical hazards while cleaning up and maintaining milling machines at the CNC milling department.     Vero Machine Industries Inc.  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 714698, citation number 1, item number 1 and was affirmed as a final order on 1/10/2013, with respect to a workplace located at 5400 85th St, Vero Beach, FL 32967.

29 CFR 1910.95(d)(1): When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program.  a.  On or about April 24, 2013, at the above addressed jobsite. The employer did not assess the noise levels or implement a monitoring program at the twin spindle lathes department for protecting employees who were exposed to noise in excess of 85 decibels during the manufacturing operations.   Vero Machine Industries Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 714698, citation number 2, item number 1 and was affirmed as a final order on 1/10/2013, with respect to a workplace located at 5400 85th St, Vero Beach, FL 32967.

On or about April 24, 2013, at the above addressed jobsite. Two circuit breakers installed inside a panelboard (QD Load Center Cat No. QOC442L) were not identified, which exposed employees to an electrical hazard.
