29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Versa-Pak, Ltd., located at 500 Staeger Rd., Celina, OH 45822: On or about October 30, 2012, the employer employer failed to properly train employees to lockout/tagout Line 3 40 bag machine (manufactured by Gamma Machinery; Model 9418SV; Serial # 2961-10) and Line 3 winder (manufactured by Gloucestrer Engineering Co., Inc.; Model 123; Serial # 27089-18) before employees were conducting activities such as, but not limited to, changing set-up, adjusting settings, or unjamming it. Employees were exposed to the unexpected startup of the machine's rotating parts while conducting work that could result in caught-in injuries. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Versa-Pak, Ltd., located at 500 Staeger Rd., Celina, OH 45822: On or about October 30, 2012, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as rotating shafts of Line 3 winder (manufactured by Gloucestrer Engineering Co., Inc.; Model: 123; Serial # 27089-18), exposing employees to caught-in hazards. Employees were at risk of injury when working in immediate proximity to rotating shafts. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
