29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Versa-Pak, LTD on or about July 9, 2024, at the location of 500 Stager Rd. Celina, Ohio 45822: The employer did not ensure Line 4 winder machine was adequately guarded to protect operators from in running nip points and pinch points. Employee tasked with "Flipping Rolls" in this operational condition were exposed to caught in hazards created between each the pneumatic roller chuck, the in-running nip of the finished product, the layon rollers, and the winder rollers. b) Versa-Pak, LTD on or about July 15, 2024, at the location of 500 Stager Rd. Celina, Ohio 45822: The employer did not ensure Line 11 winder machine was adequately guarded to protect operators from in running nip points and pinch points. Employee tasked with "Flipping Rolls" in this operational condition were exposed to caught in hazards created between each the pneumatic roller chuck, the in-running nip of the finished product, the layon rollers, and the winder rollers. c) Versa-Pak, LTD on or about July 15, 2024, at the location of 500 Stager Rd. Celina, Ohio 45822: The employer did not ensure Line 16 winder machine was adequately guarded to protect operators from in running nip points and pinch points.
