29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Spartan Chemical Company, Inc., Maumee, Ohio:  On or about November 1, 2012, the employer did not ensure that two rotating arms inside of the filler cabinet on the Packaging Dynamics manufactured Filler (Serial #M-03164) were guarded to prevent employees working in the area from accessing that danger zone of the machine while it was operating.  Employees were exposed to the hazard of being caught-on the arms as they rotated.     b) Spartan Chemical Company, Inc., Maumee, Ohio:  On or about November 1, 2012, the employer did not ensure that auger, which feeds bottles into the Packaging Dynamics manufactured Filler (Serial #M-03164), was guarded to prevent employees from becoming caught-on the rotating device.      c) Spartan Chemical Company, Inc., Maumee, Ohio:  On or about November 1, 2012, the employer did not ensure that the Hamrick Case Packer (Serial #03-1385) was guarded to prevent employees from accessing the hazard area of the machine while it was operating, exposing employees to crushing hazards.
