29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    A)    Spartan Chemical Company, Inc., located at 1110 Spartan Drive, Maumee, OH 43537: On or about June 2, 2014, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of equipment such as, but not limited to, the Bowl Cleanser Lines Kaps-All Capper (Model C8, Serial # 5953). Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic while performing maintenance activities such as, but not limited to, changing a clutch. Employees were exposed to amputation hazards.      B)     Spartan Chemical Company, Inc., located at 1110 Spartan Drive, Maumee, OH 43537: On or about November 7, 2014, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of equipment such as, but not limited to, the Bowl Cleanser Lines E-Pak Capper (Drawing # 4064-086A, Serial #0104-7999-216-205). Employees did not control all hazardous energy sources such as, but not limited to, electrical while performing activities such as, but not limited to, cleaning, changing set-ups, and replacing parts. Employees were exposed to amputation hazards.      C)     Spartan Chemical Company, Inc., located at 1110 Spartan Drive, Maumee, OH 43537: On or about June 2, 2014, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of equipment such as, but not limited to, the Hamrick Off-Packer Line (Model 300D, Serial # 11-1659). Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic while performing activities such as, but not limited to, adjusting the photo eye. Employees were exposed to amputation hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    A)    Spartan Chemical Company, Inc., located at 1110 Spartan Drive, Maumee, OH 43537: On or about November 7, 2014, the employer failed to properly train employees to lockout/tagout equipment such as, but not limited to, the Bowl Cleanser Lines Kaps-All Capper (Model C8, Serial # 5953), the Bowl Cleanser Lines E-Pak Capper (Drawing # 4064-086A, Serial #0104-7999-216-205), and the Hamrick Off-Packer Line (Model 300D, Serial # 11-1659) before employees conducted activities such as, but not limited to, cleaning, changing set-ups, replacing parts, and making adjustments. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
