29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    A. Red Oak Construction worksite located at 900 Interstate Drive, Findlay, Ohio: On or about November 1, 2012, the employer did not ensure employees wore proper eye protection while operating a pneumatic nail guns while performing framing activities. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

A. Red Oak Construction worksite located at 900 Interstate Drive, Findlay, Ohio: On or about November 1, 2012, the employer did not ensure employees were protected from head injuries while working below employees installing wood rafters. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

Red Oak Construction worksite located at 900 Interstate Drive, Findlay, Ohio: On or about November 1, 2012, the employer did not ensure an employee's personal fall arrest system was secured (clipped-off) to a Genie Aerial Lift. The exposed employee was performing framing activities from the aforementioned aerial lift located near the southeast corner of the structure.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    A. Red Oak Construction worksite located at 900 Interstate Drive, Findlay, Ohio: On or about November 1, 2012, the employer did not ensure employees were protected from falling approximately 12.00 feet working on the exterior top plate near the southeast corner of the strucuture. The exposed employee was installing trusses on the wood framed commercial structure.    B. Red Oak Construction worksite located at 900 Interstate Drive, Findlay, Ohio: On or about November 1, 2012, the employer did not ensure employees were protected from falling approximately 12.00 feet while working on the bottom cord of the previously installed trusses.
