29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: A. Red Oak Construction worksite located at 7005 Salisbury Rd., Maumee, Ohio: On or about June 17, 2013, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns when performing framing activities. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners. Red Oak Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.95, which was contained in OSHA inspection number 717479, citation number 1, item number 1 and was affirmed as a final order on March 22, 2013, with respect to a workplace located at Findlay, Ohio.

Red Oak Construction worksite located at 7005 Salisbury Rd., Maumee, Ohio: On or about June 17, 2013, the employer did not ensure employees were protected from falling approximately 24.00 feet while installing roof sheeting on the West side of the residential structure. B. Red Oak Construction worksite located at 7005 Salisbury Rd., Maumee, Ohio: On or about June 17, 2013, the employer did not ensure employees were protected from falling approximately 11.00 feet while installing roof sheeting on the West side of the residential structure located above the garage. Red Oak Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501, which was contained in OSHA inspection number 717479, citation number 1, item number 4 and was affirmed as a final order on March 22, 2013, with respect to a workplace located at Findlay, Ohio.
