29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee, who wore prescription lenses while engaged in operations that involved eye hazards, used protection that incorporated the prescription in its design, or wore eye protection over the prescription glasses or protective lenses with side shields. On or about November 1, 2012, An employee wore prescription lenses that were not ANSI approved safety glasses while working on a Mazak VTC-41 mill machine exposing his eyes to airborne objects. The Radke Machine & Tool, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.133(a)(3), which was contained in OSHA inspection number 311946354, Citation Number 1, Item number 1 and was affirmed as a final order on October 15, 2008, with respect to a workplace located at 607 NE 4th Street, Hubbard, TX 76648.

On or about November 1, 2012, the KBC horizontal band saw was not equipped with an adjustable guard, exposing employees to the hazard of amputations. The Radke Machine & Tool, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 311946354, Citation Number 1, Item number 2 and was affirmed as a final order on October 15, 2008, with respect to a workplace located at 607 NE 4th Street, Hubbard, TX 76648.
