29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. The employer failed to develop and implement written lockout/tagout procedures for each type of machine located in the facility to include but not limited to; type, manufacturer, location, energy sources, shutdown and re-energization procedures. Authorized employees locking out CNC lathes were exposed to electrocution and amputation hazards.

29 CFR 1910.147(f)(2)(i): Whenever outside personnel were engaged in activities covered by the scope and application of this standard, the on-site employer and the outside employer did not inform each other of their respective lockout or tagout procedures. The employer did not inform outside contractors performing maintenance on CNC lathes of the specific lockout/tagout procedures associated with each type of equipment.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: At the facility, authorized employees who perform energy control lock-out/tag-out were not formally trained on the program or energy control procedures for each type of machine exposing the employees to amputation and electric shock hazards.
