29 CFR 1904.29(b)(3):     Each recordable injury or illnesses was not entered on the OSHA 300 log and/or incident report (OSHA Form 300A or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. The injuries include, but are not limited to the following cases:    a) U.S. Army, Fort Gordon, GA - On 6/8/12, an employee experienced an injury to the lower back while lifting a patient. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    b) U.S. Army, Fort Gordon, GA - On 6/23/12, an employee experienced an injury to the right leg, hip and ankle after rolling right ankle while stepping out of a fire truck. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    c) U.S. Army, Fort Gordon, GA - On 7/16/12, an employee experienced a needle stick after giving an injection. The needle stick had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    d) U.S. Army, Fort Gordon, GA - On 8/2/12, an employee experienced an injury to the lower back and right knee after slipping on greasy floor. The 2injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    e) U.S. Army, Fort Gordon, GA - On 8/13/12, an employee experienced an injury to the lower back while lifting a patient. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    f) U.S. Army, Fort Gordon, GA - On 8/15/12, an employee experienced an allergic reaction from spray solution. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    g) U.S. Army, Fort Gordon, GA - On 8/22/12, an employee experienced an injury to the abdomen after lifting a toy. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    h) U.S. Army, Fort Gordon, GA - On 9/18/12, an employee experienced an injury to the right shoulder while lifting a large canine in a cage. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    i) U.S. Army, Fort Gordon, GA - On 9/19/12, an employee experienced an injury after slipping on wet floor. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    j) U.S. Army, Fort Gordon, GA - On 10/2/12, an employee experienced an injury to the right shoulder while lifting communication equipment. The injury had not been recorded in the OSHA 300 log for 2012, on or about 11/2/12.    The U.S. Army was previously cited for a violation of this OSHA standard or its equivalent standard which was contained in OSHA inspection 311095004, citation 1, item 2, issued on 4/14/2011.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
