The employer did not ensure the entrance/exit door located on the east wall of shop was not blocked by work materials and fan.  Exposing employees to burn hazards.

29 CFR 1910.134(c)(1):  A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     Colorado Custom's Granite and Marble, 8602 Ash Avenue #6, Lubbock, Texas.  On or about January 31, 2013, and at times prior thereto, the employer did not establish and implement a written respiratory protection program.  Employees were wearing MSA respirators while grinding and sanding on granite slabs.  Employees were exposed to potential sliica dust.    The Colorado Custom's Granite and Marble, was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 315277798, citation number 1, item number 1a and was affirmed as a final order on 02/14/2012, with respect to a workplace located at 8602 Ash Avenue #6A, Lubbock, Texas 079404.

On or about January 31, 2013, and times prior thereto, the employer did not ensure that employees who wore MSA respirators were evaluated medically.  Employees wore the respirators while sanding and grinding the sink openings in granite slabs.  Employees were exposed to potential silica hazards.    The Colorado Custom's Granite and Marble was previously cited for a violation of this occupational safety and health standard or its equivalent standard in OSHA inspection number315277798, citation number 1, item number 1b, and was affirmed as a final order on 02/14, 2012, with respect to a workplace located at 8602 Ash Avenue #6A, Lubbock, Texas 79404.
