29 CFR 1926.404(f)(6):     Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.    a)	On or about November 6, 2012; at the center of the jobsite, employees were allowed to utilize an extension cord that was missing a ground pin.  The extension cord was plugged into a 2 ft. multi-outlet cord that energized a miter saw used to cut metal studs.  The missing ground pin prevented the extension cord from having a continuous path to ground.
