29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) 755 Veterans Blvd., Metairie, LA; On or about 10/21/14, the employer failed to ensure two employees wore fall protection while working from an aerial lift bucket. The employees were applying sheathing to the exterior of a building. The employees were approximately 26 feet above the ground.
