29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: a) At Waterford Subdivision Lot 37 Daphne, Alabama: On or about November 7, 2012 and at times prior; the employer exposed his employees to fall hazards in that he did not ensure a homemade wooden scaffold had an adequate walking / working surface of at least 18 inches in width exposing his employees to fall hazards of 8 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At West Side of House: On or about November 7, 2012 and at times prior; the employer exposed his employees to fall hazards while working on a 10 / 12 pitch roof in that he did not ensure they had fall protection exposing them to falls of 12.2 feet to the ground below. b) At North Side of House: On or about November 7, 2012 and at times prior; the employer exposed his employees to fall hazards while working on a 10 / 12 pitch roof in that he did not ensure they had fall protection exposing them to falls of 12.5 feet to the ground below. b) At South Side of House: On or about November 7, 2012 and at times prior; the employer exposed his employees to fall hazards while working on a 10 / 12 pitch roof in that he did not ensure they had fall protection exposing them to falls of 12 feet to the ground below.
