29 CFR 1926.403(b)(2):Listed, labeled, or certified equipment was not used in accordance with instructions included in the listing, labeling, or certification: (a) 71 Colton St Springfield, MA: On or about October 18, 2012, a portable ground fault circuit interrupter, which was receiving power through an extension cord, was observed in use which is contrary to the instructions printed on the body of the device to use extension cords on the GFCI output only.
