29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder:    This violation occurred on November 6, 2012, at a jobsite located at 265 W. Pleasant Run Road, Cedar Hill, Texas 75104; where at least seven (7) employees were using an extension ladder that did not extend at least 3-feet above the upper landing to access/exit the roof, employees were exposed to falling to the ground level below.

Pleasant Run Road, Cedar Hill, Texas 75104; where at least seven (7) employees used a defective extension ladder that had not be taken out of service to access and exit the roof and were exposed to falling to the ground level below.
