29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. Employees working from a 5 in 12 pitch roof were exposed to falls of approximately 17feet to a lower level. The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Employees working to install roofing materials on a 5 in 12 pitch roof were exposed to falls of approximately 17 feet to a lower level. Employees were not protected from falling by guardrail systems, safety net systems, or a personal fall arrest systems, or any other approved means.
