29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   The employer did not ensure fall protection was implemented for employees installing asphalt shingles on a steep pitched 15:12 roof.  Employees were exposed to hazards associated with an approximately forty (40) foot fall.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  The employer did not ensure employees used the attachment point of the body harness located on the wearer's back near shoulder level.  Employees connected the rope lanyard to the front leg straps of the body harness for use as a positioning device on the steep 15:12 pitched roof.
