Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. In the fenced in area where a water tower was being painted, the extension cord used to supply power to the swing stage scaffold was cut through the insulation and into the wiring thereby exposing employees to a contact hazard. b. On the site where a water tower was being painted, the extension cord supplying power to the swing stage moters was pinched between the door and crimped from the pressure of the door on the cord thereby exposing employees to a contact hazard.

29 CFR 1926.502(e)(7): Snaphooks shall be sized to be compatible with the member to which they are connected to prevent unintentional disengagement of the snaphook by depression of the snaphook keeper by the connected member, or shall be a locking type snaphook designed and used to prevent disengagement of the snaphook by the contact of the snaphook keeper by the connected member. As of January 1, 1998, only locking type snaphooks shall be used. a. On the site where an employee was observed free climbing a ladder approximatily 120 feet, the snap hooks being used as part of the fall protection system were not designed form the climbing method of climbing and clipping while climbing the ladder thereby exposing employees to a fall hazard.
