29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses. The employer did not ensure that employees exposed to splashes of chemicals such as but not limited to sulfiric acid (corrosive), chromic acid (corrosive) and Enplate NI-431A (severe irritant) when chrome plating wore adequate eye protection in that employees wore regular prescription glasses with no side shields.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. The employer did not ensure that employees exposed to corrosive chemicals such as but not limited to sulfuric acid (corrosive), chromic acid (corrosive) and Enplate N1-431A (severe irritant) had an emergency shower in the workarea when plating hexavalent chromium.

29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. The employer did not ensure employees required to wear dust masks when polishing were fit-tested prior to the initial use.
