A periodic inspection of the energy control procedure of each authorized employee had not been completed on at least an annual basis.

29 CFR 1910.178(g)(2):     Facilities shall be provided for flushing and neutralizing spilled electrolyte, for fire protection, for protecting charging apparatus from damage by trucks, and for adequate ventilation for dispersal of fumes from gassing batteries.    In the Warehouse portion of the facility, an Exide electric battery charger was located on the floor against the East wall and and not protected from damage from a forklift truck operating in the area.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    In the Warehouse portion of the facility, a maintenance employee was exposed to an amputation hazard while using a Kalamazoo Model 8C-0, serial number 8353, horizontal bandsaw machine in that eight inch of the unused portion of the saw blade was unguarded.

29 CFR 1910.219(f)(3):     Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.    In the Polisher Room, an chrome plater was exposed to the in-running nip points of the chain and sprocket while holding metal parts being polished.

29 CFR 1910.332(b)(1):     Practices addressed in this standard. Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.    An employee who performs energized electrical work in the form of troubleshooting on 110 Volt AC to 240 Volt AC electrical circuits has not received training on what and when to wear personal protective equipment to protect themselves against electric shock and arc flash.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.    In the hallway leading to the Polisher Room, there was damage to the outer insulation, a small 1/2 inch cut on the outer insulation next to the attachment plug on a 110 Volt AC power cord for a temperature controller.
